If you have end stage heart failure with a 10%infraction rate does that mean you have 6 months to live?

my husband was diagnosed with end stage heart failure with a 10% infraction rate.he still uses cocaine and drinks alcohol.does this mean his life expectancy is 6 months or less?the doctor said he may have to get a traansplant to survive.
